Home and Community Based Services (HCBS) provide critical support for children with serious emotional disturbance, medical fragility, or developmental delays — including children in foster care. These services are designed to keep children safe, supported, and thriving in their homes and communities rather than in institutional settings.
All HCBS services are fully covered by Medicaid at no cost to your family.
Services we provide
Respite Services — trained caregivers provide temporary relief for parents and guardians, giving you time to rest while your child is safe and engaged
CFASS (Caregiver Family Advocacy Support Services) — a dedicated advocate helps your family navigate systems, access resources, and build skills to support your child
Care Management — coordination of all medical, educational, and social services your child needs
Supporting the whole family. HCBS recognizes that supporting caregivers is just as important as supporting the child. Respite and family advocacy services help prevent burnout and keep families strong.
Who qualifies
Serious emotional disturbance (SED) — children with significant mental health challenges that affect daily functioning
Medical fragility — children with complex medical needs requiring ongoing care and coordination
Developmental delays in foster care — children in the foster care system who need additional developmental support
