We accept all Medicaid managed care organizations, so no matter what plan your child is on, we can help.
What our care managers do
Navigate doctors and specialists — schedule appointments, coordinate between providers, ensure follow-through
Arrange transportation — coordinate Medicaid transport to medical appointments and therapies
School support — advocate for IEPs, 504 plans, and appropriate classroom placement
Connect to counseling — link your child with mental health services when needed
Respite and family support — identify respite care and caregiver resources to prevent burnout
In-home visits. Our care managers come to your home to meet with your family, understand your child's needs, and build a plan that works for your life.
